Sending in samples to Turf Diagnostics & Design?

Use one of the sample submittal forms below to provide the lab with the information needed to perform your tests and report your data.  A completed submittal form should be included with each sample shipment to the lab. 

If you are an international client, please remember to attach a copy of our soil import label to the outside of the shipping container or box.  This will ensure that the samples can clear through customs.

E-Submittal Form - computer fillable form.  Information can be entered from your computer, printed, emailed to lab, and/or saved for future use.  Works best with adobe reader or acrobat version 8 or newer.  Adobe reader is available free at the adobe web site.

Standard pdf Submittal Form - printable form that can then be filled out by hand.

Green Roof Submittal Form - printable form for customers in need of green roof testing.

Html Submittal Form - printable form for those that do not have the adobe reader.
